Support Worker

The potential candidate will be involved in developing people with learning disabilities to reach their maximum potential as individuals.

Responsibilities include:

• Act as Key Worker for individual service users
• Effectively assess individual needs and deliver an appropriate service
• To contribute to the individual care planning process for people using the service by written and verbal communication
• To ensure that appropriate records are kept of the user’s progress and activity within the service and that these are shared with the individual
• To share in the day-to-day work with service users which will involve assisting with personal care tasks, etc.

The potential candidate must have the ability to communicate effectively with other professionals, colleagues, service users and families/carers and have the ability to support individuals with their personal and intimate needs in a respectful and dignified manner.
